Please call 815-676-4688 to set up an appointment. If you don't know which therapist you would like to see, one of our clinicians can determine the therapist who will best suit your needs. If you reach our voicemail, clinicians are either busy with clients or out of the office. Please leave a detailed voicemail and one of our therapists will return your call promptly.
After you have confirmed an appointment with your therapist to assist in understanding your background and needs, we ask that you complete the Client History and Insurance form below.
Sessions/Meetings
During your initial counseling sessions, your therapist will gather your history, evaluate your concerns, and create a treatment plan.
Sessions are typically once a week and last 45-55 minutes per session. The number of sessions will be determined by your progress toward meeting your goals.
If you are unable to keep your appointment, please be sure to give 48-hour notice to avoid a missed session fee.
Billing/Insurance
Services may be covered in full or in part by your health insurance carrier or employee assistance program. Please contact your carrier or us for further information.
We currently accept BlueCross BlueShield PPO and privately paying clients. Some clinicians may be in network with other insurance plans, please click here to check their bio for any additional plans they are in network with.
Unfortunately we are out of network with Medicare, Medicaid, and AllKids.
Services may be billed to your insurance carrier or privately paid and a Good Faith Estimate would be provided. We accept Visa, MasterCard, Discover and American Express, as well as, check and cash.
Confidentiality is extremely important to us; we adhere to all ethical and government standards. See our Notice of Privacy Practices below.
Your information and what is discussed in session will be kept between you and your therapist. In your first session, any limits to confidentiality will be explained.
To maintain the highest level of confidentiality, each clinician is responsible for their own clinical work, records maintenance, and professional conduct, not Plainfield Counseling Center, PLLC, Naperville Aurora Counseling Center, PLLC, or any of its other clinicians. Requests for records must indicate which clinician was the provider
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost. Under the law, health care providers need to give patients who don’t have health care insurance, who are not using their health care insurance, or are using out of network insurance benefits an estimate of their bill for health care items and services before those services are provided.
If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill. Please contact your provider for questions regarding your bill.
